Terms and conditions:

1.    Limitations: This membership is not for emergency medical issues. Members are advised to seek emergency medical care for urgent health concerns.

A medical emergency is any situation that poses an immediate risk to a person's life or long-term health. Here are some examples of conditions that are typically considered medical emergencies:

  1. Heart Attack: Symptoms may include chest pain, shortness of breath, nausea, sweating, and pain or discomfort in the arms, back, neck, or jaw.

  2. Stroke: Signs may include sudden numbness or weakness in the face, arm, or leg (especially on one side of the body), confusion, trouble speaking or understanding speech, sudden trouble seeing in one or both eyes, difficulty walking, dizziness, or loss of balance or coordination.

  3. Severe Bleeding: This could be from a wound, injury, or internal bleeding and may require immediate medical attention to stop the bleeding and prevent shock or other complications.

  4. Severe Allergic Reactions (Anaphylaxis): Symptoms may include difficulty breathing, swelling of the face, lips, tongue, or throat, rapid heartbeat, dizziness, or loss of consciousness.

  5. Severe Trauma or Injury: This could include major injuries from accidents, falls, or other traumatic events that require immediate medical attention.

  6. Severe Burns: Burns that cover a large area of the body, are deep, or involve the airway can be life-threatening and require emergency medical care.

  7. Poisoning: Ingestion or exposure to toxic substances can lead to serious illness or death and requires prompt medical treatment.

  8. Severe Difficulty Breathing: This could be due to asthma attacks, severe respiratory infections, or other respiratory emergencies that require immediate intervention.

  9. Loss of Consciousness: Fainting or sudden loss of consciousness without a clear cause should be evaluated by a healthcare professional immediately.

  10. Seizures: Seizures that last longer than five minutes, occur in clusters, or are accompanied by injuries, difficulty breathing, or other serious symptoms require emergency medical attention.

It's important to note that this list is not exhaustive, and any condition that causes significant concern or requires urgent medical attention should be treated as a medical emergency. When in doubt, it's always best to err on the side of caution and seek immediate medical help.

2.    Mental Health Care Exclusion: This total wellness membership does not replace care from a licensed mental health professional. Members experiencing mental health concerns or requiring psychological evaluation, diagnosis, or treatment are encouraged to seek assistance from qualified mental health professionals. The chiropractic services provided under this membership are not a substitute for mental health care, and the chiropractor is not trained or licensed to provide mental health diagnosis or treatment. Members are responsible for seeking appropriate mental health support as needed.

3.    Primary Care Physician Exclusion: This total wellness membership does not replace the role of a primary care physician (PCP). Members are encouraged to maintain regular visits with their PCP for comprehensive medical care, including preventive screenings, management of chronic conditions, and treatment of acute illnesses. The chiropractic services provided under this membership are not a substitute for primary care, and the chiropractor is not a replacement for a primary care physician. Members are responsible for maintaining their relationship with their PCP and seeking appropriate medical care from them as needed.

4.    Health and Wellness Advice Disclaimer: The wellness plan offered under this membership agreement provides health and wellness advice intended to promote overall well-being and enhance quality of life. However, it is important to note that the advice provided is not medical advice and should not be construed as such. While the recommendations offered may support physical, mental, and emotional health, they are not a substitute for professional medical diagnosis, treatment, or advice. Members are encouraged to consult with a licensed medical professional for any specific medical concerns or conditions, and to follow their guidance accordingly. By participating in this wellness plan, members acknowledge that they are receiving general health and wellness advice and understand the distinction between such advice and medical advice provided by qualified healthcare professionals.

5.    Limitation of Services: The total wellness program provided under this membership agreement is designed to promote overall health and well-being through chiropractic care and related services. It is important to note that this program is not intended to replace the specialized expertise and services offered by medical specialists, including but not limited to dermatologists, ear, nose, and throat (ENT) specialists, internal medicine doctors, urologists, and other medical professionals. Members are encouraged to seek appropriate medical care from licensed specialists for any specific medical concerns or conditions outside the scope of chiropractic care. The chiropractor may provide referrals to medical specialists when deemed necessary, but members are ultimately responsible for coordinating their healthcare needs with appropriate providers.

6.    Chiropractic Adjustment Allocation: This total wellness membership provides a total of 17 chiropractic adjustments to be used throughout the year. Of these, 15 adjustments are included as part of the standard membership benefits, and an additional two bonus adjustments are offered as a supplement. These adjustments are to be scheduled and utilized at the discretion of the member, in consultation with the chiropractor. Unused adjustments do not roll over to the following year and expire at the end of the membership term.

7.    Membership Start Date and Scheduling Flexibility: The membership term begins on the date when the individual signs up for the total wellness program, rather than being tied to the calendar year. This ensures that members can enjoy the benefits of their membership immediately, without having to wait for the start of a new calendar year. Additionally, members have the flexibility to schedule chiropractic appointments according to their own schedule and preferences throughout the year, rather than feeling constrained by end-of-year deadlines. This approach aims to provide convenience and ease for both members and the chiropractic office, by distributing appointment requests evenly throughout the membership term and minimizing the rush of appointments typically seen at the end of the calendar year.

8.    Adjustment Usage and Membership Cancellation Policy: Members who utilize chiropractic adjustments under this membership agreement and subsequently choose to cancel their membership are subject to a fee structure based on the number of adjustments used. Specifically, each adjustment used will result in the member being charged for three weeks of membership dues upon cancellation. If a member has used all 17 adjustments and chooses to cancel, they are obligated to pay for the remainder of the annual membership term. This policy is in place to ensure fair and equitable treatment for all members and to discourage misuse of adjustments without ongoing commitment to the membership program.

9.    Acknowledgment of HIPAA Policy and Consent to Chiropractic Care: By signing up for their initial evaluation and enrolling in the total wellness membership, patients acknowledge that they have previously agreed to the Health Insurance Portability and Accountability Act (HIPAA) policy and consent to chiropractic care. The patient's agreement to the HIPAA policy and consent to chiropractic care has been documented and is on file with the chiropractic office. This acknowledgment reaffirms the patient's understanding and acceptance of these policies and their continued consent to receive chiropractic care under the terms outlined in the membership agreement.

10. Office Closure Policy: The chiropractic office may be closed on major holidays, as well as at times when the doctor needs to attend to personal matters. During these closures, chiropractic services will not be available, and scheduled appointments may need to be rescheduled. Advance notice of office closures will be provided to members whenever possible, and efforts will be made to accommodate appointment rescheduling needs. Members are encouraged to plan their appointments accordingly and to contact the chiropractic office with any questions or concerns regarding office closures.

11. Coverage Doctor In-Office Treatment Policy: At certain times throughout the year, a qualified coverage doctor may provide in-office treatments and services in the absence of the primary chiropractor. This coverage may occur for a period of 4-8 weeks annually, during which the coverage doctor will follow established treatment protocols and guidelines to ensure continuity of care for members. The coverage doctor is selected based on their qualifications and expertise in chiropractic care, and efforts will be made to inform members in advance of their scheduled appointments if they will be seen by the coverage doctor. Members can expect the same standard of care and professionalism from the coverage doctor as they would from the primary chiropractor.

12. Appointment Scheduling: Members are responsible for scheduling their appointments and adhering to the office policy for no shows and rescheduling.  Total wellness members will not be able to recoup missed appointments that are not rescheduled or canceled within the window stated in office policy after 2 missed appointments.

13. Payment: Members agree to pay the first monthly membership fee in full at the time of enrollment or according to the agreed-upon payment plan. Membership auto-renews every month unless canceled by patient.  Payments for memberships are pulled from cards that patients have provided on the 15th of every month.  If a payment does not go through, each patient will have a 5-day grace period to update their payment method without disruption of service.  After the 5-day grace period all services will be paused and the patient will lose one adjustment for every 3 weeks of non-payment.  If after 6 weeks of non-payment, the membership will be automatically cancelled.  Brewerytown Chiropractic holds the right to not re-new memberships at their discretion due to payment issues.

14. Non-transferable: Memberships are non-transferable and may not be shared with or transferred to another individual.

15. Cancellation Policy: Members may cancel their membership at any time with written notice to the chiropractic office. Cancellation requests must come from the Total Wellness Member page using the messaging tool.  Cancellations will only stop upcoming payments.  Cancellations made after payments to not receive reimbursement.

16. Health Information: Members agree to provide accurate and up-to-date health information to the chiropractor to ensure safe and effective treatment.

17. Governing Law: The membership agreement is governed by the laws of the jurisdiction in which Brewerytown Chiropractic office is located.

18.  Response Time:  Dr. Tripp typically responds to wellness inquiries and messages within 1-2 business days.  Sometimes the response will be faster.  We will do our best to not allow inquiries to go unanswered for more than 2 business days.  However, there may be circumstances that do allow some inquiries to take longer to respond to.  Our office will work hard to make these instances as infrequent as possible.
